The mandatory elements of Medicare Payment Reform put “revenue at risk” for hospitals based on clinical and quality performance. When hospitals perform well in defined areas, they receive all of “at risk” dollars they have in the program and possibly additional revenue if they perform better than their peers. If a hospital performs worse than expected in relation to its peers, it will lose “at risk” dollars and can incur additional penalties.
 
A number of measures included in The Leapfrog Hospital Survey are the same ones used in these federal programs. In order to improve performance tied to each of these programs and effectively manage “revenue at risk”, it is important for organizations to understand the connection between the various mandatory and voluntary quality and incentive programs that they participate in.
